Coverage Connection is an interactive initiative focused on engaging patients through print and online activities. This integrated approach will utilize providers, websites, emails, and social media in order to connect with the patient at several points of contact. It will offer patients a connection to an online community which promotes overall health and well-being, as well as the importance of responsible maintenance of coverage. The provider will introduce the initiative to the patient via handout. The patient will, in turn, register at the "Coverage Connection" website, which will then allow the patient to receive reminder emails to re-certify coverage and update any change in residence. In addition, an online community on social media will provide health and well-being tips, as well as posts to the patient's media stream reminding him or her to re-certify as necessary. Often, a lack of information leads to neglected responsibilities, so providing patients with the information of why coverage is important, how easily it can be lost, as well as how to maintain coverage, will result in better health outcomes for participants.
